When
filling out the USBC membership application form, please update your
address if it is not correct to insure that USBC will be able to send
your membership card to your home. Also correct/enter your birth date
(including the year) to insure that if
you qualify for awards in your age group that you will receive them.
USBC membership application forms plus required fees should be forwarded to
the Association Manager no later than 30 days from the date the form was
completed. |

Remember ladies, gentlemen and youth if you have moved since you purchased
your last USBC card then send a Change of Address
form to GFUSBCA at the above address with your correct address. A
link to the form to be used is located below. Thank you

If you have lost your USBC card,
you can go to BOWL.COM, click on find a member, enter
your name or USBC ID number and print your card on your printer or you can
call USBC Headquarter at 1-800-514-2695 and ask them to send you a
replacement. |

Need a USBC or local form, check below for a link to the required form
(forms cannot be completed on line, you must print it and fill in the
required information). Remember award forms should be forwarded to the
Association Manager no later than 20 days after the award score was rolled.
